I am a sole trader & the owner of 7Lilies, which is registered at Store 1A, Photon House, Percy Street, Leeds, LS12 1EG. I take privacy very seriously & use your data as explained further in this policy. I am the ‘Controller’ of the personal data you provide me.
Personal data is any information which can be used to identify you personally e.g. Your name, Date of birth.
I only ask for the basic contact information like your name, phone number, email address, address so that I can contact you about the services/treatments I provide you, to arrange/rearrange appointments and for client/patient feedback following a treatment.
You are required to complete a full medical/health history form for my records to fulfil insurance requirements.
Your data will be used to contact you via text, phone call, social media/messenger, email to arrange/rearrange or cancel appointments to discuss treatment needs for client/patient feedback following treatment. It will not be shared with a 3rd party unless written consent is obtained from yourself. No banking details will be kept by myself.
N.B Your personal data will only be shared if I am under a duty of disclosure to share that data in order to comply with any legal obligation or to protect your rights.
Your personal data is only stored on an encrypted pen drive and a backup encrypted pen drive which will only be accessible by me. It will be kept for up to 10 years for the purpose in which it was provided & in accordance with insurance requirements.
Any personal information no longer required will be destroyed by either formatting (wiping) of the pen drives or if any reputable shredding software is available by that.
You have the right to access your personal data that is held on you.
